Dear Settlers Life beneficiary:

Please accept our condolences on your loss of your loved one.  Rest assured that Settlers Life is here to assist you in your time of need.  We know that at the time of a loved one's death, having life insurance proceeds quickly and readily available to pay for final expense can help relieve the stress of an emotional event.

To start the claim process, you may call our Claims Department at 1-800-334-9607 for a claim form, or you can simply click here for one.

The claim form will need to be completed, and signed by the appropriate beneficiary(s)*.  Please return all originals to us, along with a certified death certificate with cause of death and the applicable policy(s). 

If the beneficiary(s) wishes to assign the policy proceeds to a funeral home, in addition to the above claim form, we will require a signed, notarized assignment form.   Simply give the assignment form to the funeral home director for completion.  Please note the beneficiary(s) must sign the assignment for policy proceeds to be paid to the funeral home.  Click here for an assignment form.

If additional information is needed, the Claims Department will contact the beneficiary(s) directly.  Likewise, after the claim is processed, the beneficiary(s) will be notified by mail.
