Request For Service
The Request for Service Form will allow you to request a copy of a lost policy as well as change your name and address, mode of payment, and beneficiary.
To submit a Request for Service form, please follow these instructions:

- Print the form and fill in the sections that are appropriate to your change.
- Please make sure you've signed the form and then mail it to the address at the top of the form.
- You will receive a letter confirming the changes you requested.
Note: In order to make these changes, we require a signature; this is for your protection and for this reason, we need to have you print, sign and mail this form to us.
